Account holders must regularly verify/update contact information on file to ensure regular and timely communications from the department. This includes, but is not limited to, any changes made to the following:
- Email address
- Physical address
- Phone number
- School type (i.e., independent home school, private school)
- LEA of residence
- Change in legal status (i.e., conservatorship or Power of Attorney documentation)
If at any time the student and/or account holder information submitted in the IEA application and/or contract changes, the account holder must report the changes by submitting this form to the department within seven calendar days of the change being effective. Failure to provide this information to the department by the deadline will result in payment delays and/or suspension of the IEA account or removal of the student from the IEA Program.
Please Note:
- For any address changes, the following documentation must be submitted to the department with this form:
- Proof of residency showing the account holder's name and physical address (e.g., a copy of most recent utility bill, current rental agreement, current property tax bill, affidavit from landlord, or most recent mortgage statement); AND
- a NEW W-9 form listing the account holder’s current name and physical address. The W-9 form is posted on the forms tab of the IEA Webpage.
- If the legal name of the student and/or account holder has changed, a copy of the legal document approving the name change (e.g., marriage certificate) must also be submitted to the department with this form.
